At Thrive Dental, we believe in preserving your natural teeth whenever possible. Extraction is a last resort, not a first choice. We explore root canal therapy, crowns, gum disease treatment, and other restorative options before recommending extraction. But when a tooth truly cannot be saved, we perform extraction skillfully, gently, and with full attention to your comfort and healing.
When Extraction Becomes Necessary
Tooth extraction is indicated when:
- Severe Decay: Decay has advanced so far that the tooth structure cannot support a crown or filling.
- Advanced Periodontal Disease: Bone loss is so significant that the tooth is mobile or cannot be stabilized.
- Fractured Teeth: The tooth is fractured below the gum line where repair is impossible.
- Impacted Wisdom Teeth: Third molars are stuck under gum or bone, causing pain, infection, or crowding.
- Failed Root Canal: A tooth that had root canal therapy is reinfected and cannot be re-treated.
- Infection or Abscess: An infection cannot be resolved with antibiotics or root canal therapy.

The Extraction Procedure
Anesthesia: We apply topical numbing gel, then inject local anesthetic around the tooth. Within 3-5 minutes, the area is completely numb.
Elevation: Using an elevator (a small lever instrument), we gently loosen the tooth from its socket by working around all sides.
Forceps Removal: We grip the tooth with forceps and gently rotate and rock it back and forth to enlarge the socket slightly, then lift the tooth straight out.
Socket Care: We clean the socket of any debris or remaining tooth fragments, then assess the bone and gum.
Blood Clot Formation: We pack the socket with gauze and ask you to bite firmly for 30-45 minutes. This allows a blood clot to form—essential for healing.
For impacted teeth or teeth with curved roots, we may perform a surgical extraction, which requires a small incision, removal of bone, and sometimes division of the tooth into sections for easier removal. Surgical extractions may require a few stitches.
Aftercare: The First 24 Hours
Bleeding Control: Bite on gauze for 30-45 minutes. If bleeding continues, replace with fresh gauze and bite for another 30 minutes. Minor oozing for 24-48 hours is normal.
Swelling Management: Apply ice packs (20 minutes on, 20 minutes off) for the first 24 hours. Swelling peaks at 2-3 days, then improves.
Pain Control: Take prescribed pain medication or over-the-counter pain reliever (ibuprofen or acetaminophen) as needed. Thrive Dental provides detailed instructions before you leave.
Diet: Eat soft, cool foods (smoothies, ice cream, yogurt, mashed potatoes, soup). Avoid hot foods and drinks, crunchy foods, and spicy foods that may irritate the socket.
Avoid These for 24 Hours:
- Rinsing your mouth
- Spitting forcefully
- Drinking through straws
- Smoking
- Vigorous exercise
After 24 Hours: Gently rinse with warm salt water (1/2 teaspoon salt in 8 oz water) after meals and before bed to keep the socket clean.
Healing Timeline
- Days 1-3: Swelling and mild discomfort are normal. Healing tissue forms a dark-colored clot—don’t disturb it.
- Days 4-7: Swelling decreases significantly. Discomfort improves.
- Weeks 2-4: Gum tissue continues filling the socket. Surface healing is largely complete. Initial soft tissue healing takes 7-10 days.
- Weeks 4-12: Bone gradually fills the empty socket. Full bone healing takes several weeks to complete.
- Months 3-6: Bone remodeling continues—this is not visible but is important for long-term jaw structure.
Most patients feel significantly better by day 5-7. Initial healing of soft tissues takes 7-10 days, but complete bone healing may take several weeks or longer.
